This course provides hospice clinicians and quality leaders with an in-depth overview of the new Hospice Quality Reporting Program (HQRP) measures for Timely Follow-Up of Pain Symptoms and Timely Follow-Up of Non-Pain Symptoms. Participants will learn how these HOPE-based process measures capture responsiveness to symptom impact, what triggers a Symptom Follow-Up Visit (SFV), and how to document compliance within required timeframes. The session also outlines measure exclusions, data collection requirements, and strategies to improve documentation and interdisciplinary coordination to enhance hospice quality scores.

This course is self-paced so each participant is able to complete the module as their time allows.
