Social Determinants of Health Impacting Your Outcomes
Social Determinants of Health (SDoH) are part of the Centers for Medicare and Medicaid Services (CMS) initiative for health equity. New OASIS-E items can be used to screen your patients for increased risk of acute care hospitalization. Zeroing in on the data obtained from these items can lead to more individualized plans of care (POCs) leading staff to prioritize care, keep patients out of the emergency department (ED) and acute care hospital, and increase patient satisfaction.
Join our McBee experts as they dive into SDoH and thitsmpact on patient outcomes.
